Instructions:

  1. To participate in the survey, open your web browser and type in this URL https://www.tellofficedepot.com/ or www.OfficeDepot.com/feedback.
  2. Enter your survey code in the field with the words ‘survey code’ next to them. You can find the survey code at the bottom of your receipt.
  3. Next, enter the time. Do not enter the current time. Enter the time as is printed on your receipt. You can find it at the top of the receipt below the large OfficeMax print.
  4. Continue by truthfully answering all of the questions.
  5. Once you’ve finished answering the survey, enter your personal contact information. This information will be utilized to get in touch with you and not for anything else.
